Summary Care Records (SCR) – information for patients

Your Summary Care Record is a short summary of your GP medical records. It tells other health and care staff who care for you about the medicines you take and your allergies.

When you register with our Practice, as default, we will assume you will be happy to share your SCR. If you would like to opt-out, please click the link below with more information and opt-out form.
